DHI Hair Transplant in Turkey: Cost, Results & Recovery

How the implanter pen works, when DHI is the right choice, what it costs and how recovery goes, with real DHI cases and published sources.

DHI (direct hair implantation) is a hair transplant in which each graft is placed with a pen-shaped implanter, instead of being set into a channel opened beforehand. A DHI hair transplant in Turkey with our team takes one day under local anaesthetic, suits hairlines and areas where you still have some hair, and costs $3,350 as an all-inclusive package.

A DHI implanter pen placing a graft in the scalp, next to a row of pens loaded with grafts

DHI is often sold as a completely different operation from FUE. It is not. The grafts come out of the donor area exactly as they do in FUE; what changes is how they go back in. That one difference explains both what DHI is good at and why it makes for a longer day.

Short answer

DHI takes follicular units, small natural groups of 1 to 4 hairs, from the back and sides of your head with the FUE method. Each graft is then loaded into a Choi implanter pen and placed straight into the scalp in one movement. It works best at the hairline and between existing hair, often without shaving the top. Your graft number is set at your assessment, the operation takes 6 to 8 hours, and the final result takes 12 to 18 months. Our DHI Comfort Package costs $3,350.

What Is a DHI Hair Transplant?

DHI is named after the way the grafts are placed. In classic FUE, the doctor first opens hundreds of tiny channels in the thinning area, and the grafts are then set into them with fine forceps. In DHI, nothing is opened in advance. The implanter pen makes the opening and leaves the graft inside it, one graft at a time.

The pen itself is simple: a hollow needle with a plunger. An assistant slides a graft into the needle, the doctor inserts it at the planned angle and depth, and the plunger pushes the graft out as the needle comes back. Hair surgeons today place grafts in one of three ways: into sites made beforehand, by "stick and place", where each site is made and filled straight away, or with implanters like this one.[1]

Diagram of a DHI hair transplant: grafts extracted from the donor area, prepared in dishes, then implanted with a DHI implanter pen
From extraction to implantation: the three stages of a DHI hair transplant.

The first half of the operation is plain FUE. Follicular units, natural groups of one to four hairs, are taken one by one with a micromotor device and punches about 0.7 to 1.2 mm wide.[2][3] So DHI is less an alternative to FUE than a different way of finishing it. The medical literature describes direct implantation in exactly those terms, as a modification of FUE rather than a separate method.[4]

The Choi Pen, Long DHI and Robotic DHI

The tool most DHI teams use goes back to 1992, when two Korean doctors described the Choi hair transplanter. They used it to place single hairs: refining hairlines and rebuilding eyebrows and beards.[5] Fine, careful work at the front is still where the pen does best.

Two other names come up in searches. Robotic DHI uses a machine for part of the work; ours is done by hand. Long DHI means implanting without cutting your existing hair short, which is possible only in some cases, as our no-shave hair transplant page explains.

Who Is DHI For?

DHI earns its place where precision matters more than speed. With the pen, the doctor can slip grafts between hairs you still have, without cutting them, and set the angle of each one by hand. It tends to suit you if:

  • Your hairline or temples need rebuilding and you want a soft, slightly irregular front edge.
  • You still have hair in the area, and the aim is to add density between it.
  • You would rather not shave the top of your head, and your graft count allows it.
  • Your hair loss has slowed down, ideally with medical treatment such as finasteride or minoxidil.[6]
  • Your donor area at the back and sides is dense enough for the plan, with some reserve left for later.[2]

The hairline is drawn and agreed before the operation starts.

A large bald area is a different job. Covering a wide crown means thousands of grafts, and placing every one with a pen makes for a very long day. In those cases we often plan FUE, or FUE for the crown with DHI at the hairline. If you are not sure how far your hair loss has gone, our free Norwood test gives you a starting point.

The type of hair loss matters too. Pattern hair loss, the kind that runs in families, responds well. Alopecia areata, which causes round bald patches, is usually not treated with a transplant.[7]

DHI for Women

Women often keep their hairline and lose density behind it, so the grafts have to go in between existing hair. That is exactly what the pen is good at, and the top usually does not need shaving. Three of the six DHI cases in our gallery are women.

Diffuse thinning over the whole scalp needs a closer look first, because the donor area may be thinning as well. Our guide to hair transplants for women explains how the cause is checked before anyone talks about surgery.

DHI for Curly and Afro-Textured Hair

Curly hair curls under the skin as well, so extraction and loading both take more care and more time. Two of our DHI gallery cases are men with Afro-textured hair, one of them with 5,587 grafts. Our Afro hair transplant page explains how we plan these cases.

Benefits and Limitations of DHI

DHI has real strengths and real costs. This is the balance we walk patients through when they are choosing between DHI and FUE.

What DHI Does Well

  • Control at the front. The doctor sets the angle, direction and depth of every graft with the pen, and those details are what make a hairline look natural.
  • Less shaving. Grafts can go in between existing hair, so the recipient area can often stay long.
  • Less handling. Each graft moves from the dish to the pen to the scalp. Keeping grafts moist and cool, handling them less and shortening their time out of the body all help them survive.[4]
  • No linear scar. The extraction is FUE, so the donor area heals with tiny round dots.[3]

Where DHI Has Limits

  • It is slower. Every graft is loaded and placed on its own, so a large session takes longer than with FUE.
  • It needs a bigger team, with assistants loading pens all day. Studies that compare implantation methods measure procedure time, staff and cost, not only the result.[8]
  • Tighter packing is not always better. In a small Korean study with an implanter, 20 to 30 single-hair grafts per square centimetre survived better than 40 or 50.[9]
  • The donor supply is just as limited as in FUE.
  • It does not stop future hair loss. Your own hair can keep thinning around the new grafts.[6]

Is a DHI Hair Transplant Worth It?

It is when the case fits the tool. Rebuilding a hairline or temples, or adding density between hair you still have, are the classic DHI jobs, especially if you want to keep the top unshaved. A 2025 review of current practice describes long-lasting, natural results with high patient satisfaction, as long as hairline design and medical treatment are done well.[10]

It is less worth it for a very large bald area, where FUE usually covers more ground in a day, or while your hair loss is still moving fast. Then a staged plan, finasteride first, or FUE with DHI kept for the front may serve you better.

How DHI Is Performed, Step by Step

The operation fills one day, usually 6 to 8 hours. Around it, the whole process looks like this:

  1. Free photo review: you send photos, and the medical team suggests a technique, a graft range and a package.
  2. Arrival in Istanbul: a driver meets you at the airport and takes you to the hotel.
  3. Medical check: blood tests and a medical review come before anything else.
  4. Operation day: hairline design, extraction, pen loading and implantation.
  5. Next-day check: the dressing comes off and you learn how to wash. Most patients fly home after two or three nights; the package covers two hotel nights.
  6. Follow-up: aftercare consultations for 2 years.

1. Hairline Design

Doctor marking the planned hairline on a smiling patient before a DHI hair transplant

The doctor draws the new hairline with you sitting upright, shaping it to your face and your age, and marks the safe donor zone at the back. Nothing starts until you have looked in the mirror and agreed to the line.

2. Local Anaesthetic, With Sedation if You Want It

Local anaesthetic numbs the donor and recipient areas. Like most hair transplant patients, you stay awake.[6] Our DHI package includes sedation support for anyone who wants to feel calmer through a long day.

3. Extracting the Grafts

Grafts come out one by one with a punch on a micromotor device, spread across the donor area so it stays even. A team member sorts them by the number of hairs and keeps them cool until they are used.

4. Loading the Implanter Pens

This is the step that only DHI has. Assistants load each graft into the tip of an implanter pen and hand the loaded pens to the doctor, so the doctor can keep implanting without waiting for the next graft.

5. Implanting With the Pen

Gloved hand implanting a graft with a DHI implanter pen in the recipient area

The doctor places each graft with the pen in one movement, judging its angle and depth by hand. Single-hair grafts go at the very front for a soft edge. Grafts with two to four hairs sit further back, where they build density.

6. Dressing and the Next-Day Check

You go back to the hotel with a light dressing on the donor area. The next day the team takes it off, checks the grafts and shows you how to wash at home.

Is DHI Painful?

The part people remember is the start. Local anaesthetic goes into the donor and recipient areas, and those injections sting for a few minutes. Surgeons have several ways to make them less painful,[1] and once the skin is numb you feel pressure and movement rather than pain.[7]

A long day brings its own discomfort. Lying still for hours can stiffen your neck and back, so there are breaks, lunch included, and the anaesthetic is topped up as it wears off. Sedation helps if you are nervous.

That evening the scalp feels tight and sore, mostly at the back. A tight, achy and swollen scalp for a few days is normal,[7] and simple painkillers usually keep it under control. If the pain grows instead of fading, call the team.

DHI Donor Area and Graft Numbers

DHI grafts come from the same place as FUE grafts: the band of hair at the back and sides of the head that resists DHT, the hormone behind pattern baldness. Think of it as a budget. Each graft taken is one less for later, and taking too many from one spot, or from outside the safe zone, can leave the back of the head looking thin and patchy.[2]

How Many Grafts Can Be Harvested for DHI?

There is no standard number of grafts for a DHI hair transplant. DHI uses the same extraction as FUE, so how many grafts can be harvested depends on your degree of hair loss, the density of your donor area, the quality of your hair follicles and the hair you still have in reserve.

A small area, such as rebuilding the front hairline, may need about 1,000 grafts or fewer. Advanced hair loss at Norwood 5, 6 or 7 needs far more, and in some suitable patients the total reaches 4,000 to 5,000 grafts or more. That does not mean every donor area can give this many, or that all of them can be transplanted in a single session. For large areas the team may combine Sapphire FUE and DHI (see our 4000 grafts cost guide).

Older techniques set a hard limit. In FUT, the follicles came from a strip of skin cut from the donor area, so the number of grafts depended on the size of the strip and its density. Today a micromotor device takes follicles one by one, in a controlled and balanced way, from different points of the donor area. In suitable patients this opens up a wider part of the donor area, but a micromotor does not make the supply unlimited.

When a plan needs two sessions, they are usually at least 12 months apart, so the first result and the donor area can be judged before the second. These are typical ranges by Norwood stage, the 1 to 7 scale of male pattern hair loss:

The ranges come from our guide to how many grafts you need, and they are the same whether the grafts go in with forceps or with a pen.

Does the Donor Area Grow Back?

The follicles that were removed do not, but the hair around them keeps growing and covers the small dots. Some of your own hair may shed after surgery, which is called shock loss, and it usually returns over the following months.[2] At one year the dots tend to show only on a head shaved with a razor.

DHI Recovery Timeline

Recovery after DHI is much the same as after FUE: the extraction is identical, and the implanted area heals in a similar way. The skin settles in about two weeks. The hair takes 12 to 18 months. This is what you can expect at each stage:

WhenWhat you usually seeWhat to do
Days 1-3
A tight, red scalp; swelling may reach the forehead
Rest with your head raised and take your medication on time
Days 4-7
Small crusts around each graft, and itching
Wash gently the way you were shown; do not scratch
Days 7-14
The crusts come away and the donor area calms down
Most daily routines return; still no heavy exercise
Weeks 2-8
The transplanted hairs fall out[6]
Nothing: this is normal and the roots stay under the skin
Month 3
Your hair may look thinner than before surgery[6]
Be patient; this stage passes
Months 3-4
New hairs start to grow[7]
Small spots can appear as hairs break through
Months 6-9
Most patients see a clear change[6]
Take photos in the same light to compare
Months 12-18
The hair thickens into the final result[7]

Washing Your Hair After DHI

At the next-day check the team shows you how to wash: foam patted onto the grafts, never rubbed in. Wash this way for the first 7 to 10 days. Once the crusts have gone and the grafts are secure, you can go back to washing normally.

Going Back to Work and Exercise

Plan for one to two weeks off work. The NHS gives the same range,[7] and desk work is possible sooner if you do not mind the redness showing. For exercise, short walks are fine after a few days, the gym after weeks 2 to 4, weight training after weeks 3 to 4, and swimming or the sauna after about a month.

When Can I Get a Haircut After DHI?

A scissor trim of your existing hair is usually fine from about week 2 or 3, once the crusts are gone. Keep clippers away from the donor area for about a month, and leave the transplanted area alone until new growth starts, around month 3 or 4. Check with the team before any close shave.

The rules that matter most in the first two weeks:

  • Keep your hands off the grafts: no touching, scratching or pressing.
  • Sleep on your back with your head raised for the first nights.
  • Use only the shampoo and lotion you were given, the way you were shown.
  • Avoid sweating, strong sun, saunas and heavy lifting.
  • Do not smoke while the grafts settle.

Our hair transplant recovery timeline goes through each week in more detail.

DHI Results and Before-and-After Cases

DHI results follow the same slow rhythm as any transplant: shedding first, a quiet few months, then steady change from about month 6. Hair grows roughly 1 cm a month,[2] so length and fullness keep improving well into the second year.

What to Check in DHI Before-and-After Photos

  • The month. Judge a result at 12 months or later; at 4 months there is very little to see.
  • The graft count. 3,000 and 6,000 grafts are very different operations.
  • The angle and the light. Wet hair, a flash and a lifted fringe all change the picture.
  • The donor area. A dense front means little if the back has been thinned to get it.
  • The technique. When you compare providers, check whether the photos show manual or robotic DHI; all of ours are done by hand.

Is DHI Permanent?

The transplanted hair usually is. It comes from the back and sides, where follicles are genetically resistant to DHT, and it keeps that resistance after the move. Doctors call this donor dominance. It was first described in 1959, and current reviews still rely on it.[12][10]

What is not permanent is the rest of your hair. Your own hair can keep thinning around the transplant,[7] so a result can look thinner years later. Medication can slow that down,[6] and some people choose a second session. Our article on how long hair transplants last follows a result over the years.

Will a DHI Hair Transplant Look Natural?

It should, and the hairline is where DHI is strongest. A natural look comes from single hairs at the front and angles that follow your own growth, on a line that suits your age. When a transplant does look unnatural, the cause is usually the design: a hairline drawn too low or too straight, or grafts set at the wrong angle.[11]

Risks and Side Effects of DHI

DHI is minimally invasive, but it is still surgery, and the extraction carries the same risks as any FUE. A 2026 review found that complications after FUE are uncommon and depend both on the patient (other illnesses, smoking, some medicines) and on technique (punch design, graft handling, time out of the body).[11]

Common, Temporary Side Effects

  • Swelling that can reach the forehead in the first days
  • Redness and small crusts for about 7-14 days
  • Itching and numbness that fade over weeks or months
  • Shock loss between weeks 2 and 8
  • Small spots as new hairs break through

Less Common Complications

Some problems are uncommon but worth knowing about before surgery. Inflamed follicles (folliculitis), small cysts, pale spots in the donor area, raised scars and, very rarely, skin damage in the recipient area are all described in the literature.[11] Tell the doctor beforehand if you tend to form keloid scars. Our guide to hair transplant side effects covers each one.

Signs of Infection

Infection is uncommon, but it needs quick treatment. Contact the team straight away if you notice:

  • Yellow or green pus
  • Redness that spreads instead of fading
  • Pain that gets worse after the first few days
  • A fever

Can DHI Fail?

Complete failure is rare. There is still a small risk that grafts do not take.[7] The usual reasons are rough handling, grafts kept out of the body too long, smoking, and pressure or scratching in the first days.[11] With an implanter, packing grafts too tightly can also lower how many survive.[9] And sometimes a result disappoints simply because the native hair around it kept falling out.

DHI vs FUE and Sapphire FUE

All three take the grafts the same way. They differ in how the thinning area is prepared and how each graft goes in.

FUE vs DHI diagram: both take grafts one by one with a micromotor punch; FUE opens channels first with sapphire or metal slits and places the grafts with forceps, while DHI implants each graft with an implanter pen in one step
The extraction is the same; FUE places the grafts in two steps, DHI in one.

Sapphire FUE

None of them is better for everyone. FUE covers a large area faster; DHI gives finer control where you still have hair. Our FUE hair transplant guide explains the other side, and the FUE vs DHI comparison puts the two next to each other. Sapphire FUE differs from classic FUE only in the slit used to open the channels.

Some plans use two of them: FUE with sapphire slits for a wide crown and DHI for the hairline. FUT, the strip method, works differently again. It removes a strip of scalp and leaves one linear scar at the back, which longer hair hides.[7]

DHI Hair Transplant Cost in Turkey

With us, a DHI hair transplant in Turkey costs $3,350 as the all-inclusive DHI Comfort Package. It is a package price, not a bill per graft, so your graft number is set by your case and not by your budget. The package includes:

  • DHI with sedation support and a custom hairline design
  • Blood tests and a full medical analysis
  • A combined PRP and exosome treatment
  • An advanced aftercare kit with a serum and vitamin and mineral supplements
  • The next-day dressing and medical check-up
  • 2 hotel nights and VIP airport, hotel and hospital transfers
  • A personal medical assistant and interpreter
  • 2 years of aftercare consultation

Flights are not included. All four of our packages are compared on the treatments and packages page.

Why DHI Costs More Than Our First FUE Package

Our FUE packages start at $2,550. DHI is $3,350 because the day needs more people and more time: assistants load pens throughout, and the doctor places every graft by hand. The DHI package also includes PRP and exosome therapy as standard. It is not our most expensive option, though; the Exosapphire Microflow Elite FUE package is $4,500.

DHI Cost in Turkey vs the USA, UK and Europe

Prices abroad are published for hair transplants in general, usually per graft, and rarely for DHI on its own:

None of these figures include travel. Ours include the hotel, transfers and aftercare, but not flights. Most insurance plans do not cover a hair transplant;[13] if your hair loss follows an injury, a burn or surgery, ask your insurer in writing.

What Does "1 DHI" Mean in a Quote?

Nothing standard. Some providers use it for one session, some for one graft and some for a whole package. Ask what the price covers and how many grafts are planned. Our hair transplant cost guide explains the difference between per-graft and package pricing.

DHI Hair Transplant Reviews

The DHI reviews worth reading show the whole story, from the crusts of the first week to photos taken a year or more later. Independent platforms tell you more than any provider's own website, ours included. Our patients review us on Google, Trustpilot, Bookimed and ProvenExpert, with links on our review page, and we do not pay for reviews or reward them.

DHI Hair Transplant: Frequently Asked Questions

Is a DHI hair transplant painful?
Mostly the anaesthetic injections at the start; after that the scalp is numb and you feel pressure, not pain. The scalp is sore and tight for a few days, and simple painkillers usually help. Our DHI package includes sedation support.
Is DHI better than FUE?
Not for everyone. DHI gives finer control at the hairline and between existing hair, often without shaving the top, while FUE covers large bald areas faster. The design and the team's skill matter more than the method, and some plans combine both.
How many grafts can be implanted with DHI in one session?
There is no standard number. It depends on your hair loss, donor density, follicle quality and existing hair: a hairline alone may need about 1,000 grafts or fewer, advanced hair loss 4,000 to 5,000 or more. We set it after examining your donor area.
Does a DHI hair transplant require shaving?
The donor area at the back is usually shaved, at least in part. The recipient area can often stay unshaven, because the pen places grafts between existing hair. Fully unshaven DHI depends on your hair density and graft count; see our no-shave hair transplant page.
When can I wash my hair after DHI?
The team shows you how at the next-day check. Wash gently that way for the first 7-10 days, then normally once the crusts have gone and the grafts are secure.
How soon can I return to work after DHI?
Plan for one to two weeks off. Desk work is possible sooner if you do not mind visible redness and crusts; physical work should wait until the team clears it.
How long after DHI can I work out?
Short walks after a few days, the gym after weeks 2-4, weight training after weeks 3-4, and swimming or sauna after about a month. Heavy sweating and blood-pressure spikes can harm new grafts.
Does a DHI hair transplant leave scars?
Not a line. The extraction leaves tiny round dots in the donor area, and when they are well spaced, short hair usually hides them.
Who performs DHI at Turkey Hair Center?
Our medical team, at partner hospitals and polyclinics in Istanbul. Turkey Hair Center® is a UK-headquartered global hair restoration company: we plan your case with the team, arrange the hospital, hotel and transfers, and stay in touch through 2 years of aftercare.
What does DHI stand for?
Direct hair implantation. The name refers to placing each graft directly with an implanter pen, without opening channels first. The grafts themselves are taken with the FUE method.
Is Turkey safe for a DHI hair transplant?
It can be; what decides it is who does the work and how you are followed up. Ask who places the grafts with the pen, where the operation takes place and what aftercare you get once you are home. With us, our medical team works at partner hospitals and polyclinics in Istanbul, blood tests and a medical review come before anything else, and aftercare consultation lasts 2 years. See common hair transplant concerns for the warning signs.
Is 1 graft equal to 1 hair?
No. A graft is a follicular unit, a natural group of one to four hairs, so 3,000 grafts carry more than 3,000 hairs. Single-hair grafts go at the very front of the hairline, where the implanter pen does its finest work.
What is the difference between DHI and DHT?
DHI is a hair transplant technique; DHT (dihydrotestosterone) is the hormone behind pattern hair loss. A DHI transplant moves follicles from the back and sides, which resist DHT, into the thinning area. Our page on DHT and hair loss explains the hormone.
How much do 3,000 grafts cost with DHI?
$3,350 with us. The DHI Comfort Package has one price, so 2,000, 3,000 or 4,000 grafts cost the same, and your graft number is set by your case, not your budget. Our 3,000 grafts cost guide shows the area that number usually covers.
What happens 10 years after a DHI hair transplant?
The transplanted hair is usually still there, because it keeps the DHT resistance it had at the back of the head. The hair around it may have thinned with age, so we place the hairline for the years ahead and often suggest medication to protect your own hair. Is DHI permanent? covers this in more detail.
Is 4,000 grafts overharvesting?
Not automatically. The limit is your donor area, not a fixed number: its size and density decide what can be taken safely, and we set the count after examining it. For a large area, the team may plan Sapphire FUE for the crown and DHI for the hairline; see how many grafts can be harvested for DHI.
What are the downsides of DHI?
It is slower than FUE, it needs a bigger team to load the pens, and very tight packing is not always better. The donor supply is as limited as in FUE, and your own hair can keep thinning; see where DHI has limits.
Do you offer payment plans for a DHI hair transplant?
No, we do not offer instalment plans. You reserve your date with a $300 deposit, which is deducted from the package price and can be refunded within 7 days of payment; see deposit and payment.
