Norwood 7 Hair Transplant: Donor Limits & Realistic Results

Performed at authorized partner hospitals Turkey Hair Center is a Ministry of Health authorized health-tourism intermediary, not a clinic.

A Norwood 7 hair transplant moves hair from the narrow band left at the sides and back of the head to the large bald area on top. It is the hardest stage to plan, and it is worth saying so plainly: the donor band is at its smallest just when the bald area is at its largest. Even so, many men at this stage can get a natural, face-framing result once the plan is built around what the donor area can really give.

If you want the short answers first, here they are:

  • Can a Norwood 7 get a transplant? Sometimes yes. It depends on the density, width and quality of the remaining band, not on the stage alone.
  • Can the whole top be covered? Rarely at full density. The plan puts a limited supply where it shows most.
  • How many sessions? Usually more than one, and each session can take less than the one before.
  • What comes first? The front and the hairline. The crown is treated last or left lighter.
  • Is there an option without surgery? Yes. Scalp micropigmentation adds the look of shaved hair, alone or with a transplant.

This guide covers the procedure. If you are not sure of your stage yet, our guide to the Norwood scale shows all seven.

This man is not one of our patients and not a transplant result, and one photo cannot stage him exactly. It shows the situation this guide is about: the top is bare from front to back, while hair still grows at the sides and back. That band is all a surgeon has to work with.

Two line drawings of a head from above: stage VI with a bare top and a full band at the sides, stage VII with a narrower band set lower on the head
Stage VI (left) and stage VII (right) on the Norwood scale. At stage 7 the band of hair at the sides and back is narrower and sits lower. Illustration: Keministi, CC0, via Wikimedia Commons, cropped.

The bald area in both drawings looks similar. The difference that matters for a transplant is the band underneath it, because that band is the donor supply.

What Is Norwood 7 Hair Loss?

Norwood 7 is the last and most advanced stage of the Norwood scale, the classification of male pattern baldness that O'Tar Norwood published in 1975. The front, the top and the crown are bald as one area. Hair remains only as a horseshoe shaped band around the sides and the back of the head.

Three features set it apart from stage 6:

  • A narrower band. The hair at the sides and back covers less height than before.
  • A lower band. The upper edge of the horseshoe has moved down the head.
  • Finer hair. The hair in the band can be thinner than at earlier stages.

Norwood 6 vs Norwood 7: What Changes?

Searches for "Norwood 6 vs 7" are common, and photos online are often labelled wrongly. The top of the head can look almost the same. The band is where the stages differ.

If your band is still wide and dense, our guide to the Norwood 6 hair transplant may describe your situation better.

Why Is the Donor Area Often Not Enough?

It is a question of supply and demand. The 2021 hair transplant practice guidelines recommend that surgeons stay within the safe donor area, which they put at about 189 to 203 square centimetres depending on the person and ethnicity. At Norwood 7 that safe band is narrower than at earlier stages, so it holds fewer follicles. The bald area it has to serve is at its largest.

Rose lists the limited supply of donor hair among the main challenges in hair restoration surgery. At this stage that challenge decides the plan. The goal changes from "cover everything" to "spend every graft where it changes your appearance most".

Can a Norwood 7 Get a Hair Transplant?

Yes, some men at this stage are good candidates, and others are not. The StatPearls review of hair transplantation lists the traits of an ideal candidate: a clear and stable pattern of loss, a healthy scalp, donor hair of good quality and quantity, good general health and realistic expectations. At Norwood 7 the donor band decides most of it.

What Makes a Good Norwood 7 Candidate?

These points weigh most at this stage:

  • Donor density. The safe donor zone usually holds about 65 to 85 follicular units per square centimetre. Above 80 is excellent, and below 40 is less suitable.
  • Band width. A wider band gives more room to harvest without leaving a thin look.
  • Hair calibre. Thick hairs cover more scalp than fine ones from the same number of grafts.
  • Hair and skin contrast. Low contrast, such as grey hair on fair skin, makes thin coverage look fuller.
  • A stable pattern. Loss that has stayed the same for years is easier to plan for.
  • Expectations. The aim is a natural frame and coverage, not the density of a man who never lost hair.

A doctor makes this decision from photos and an examination. Our medical team gives a first opinion from your pictures, free of charge, and the surgeon who will operate makes the final call.

Is Quality More Important Than Quantity?

At this stage, often yes. The guideline warns that harvesting grafts with miniaturised hairs can harm the result, and that taking too much can leave a visibly depleted donor zone. It adds that diffuse hair loss should always be considered in such cases, along with medical treatment and body hair harvesting. A band that looks full but contains many fine, shrinking hairs gives less than its graft count suggests.

How Many Grafts Does a Norwood 7 Need?

There is no honest single number. Covering the hairline, mid-scalp and crown already takes about 5,000 to 6,500 grafts or more at Norwood 6, in the medical team's planning ranges, and the bald area at stage 7 is at least as large. Our guide to how many grafts you need therefore gives no fixed range for stage 7: it is planned in stages, and the donor area sets the limit.

The supply side can be measured. Chouhan and colleagues (2019) studied 580 men with androgenetic alopecia and calculated how many follicular units could be taken at 25 percent extraction from donor zones of different sizes:

Donor zone (580-man study)One sittingTwo sittings
Limited2,0643,612
Standard3,0975,420
Extended3,6126,321

A Norwood 7 band is often closer to the limited zone than to the extended one, because it is narrow and low. Read against a full-top plan, the table shows why:

  • One sitting covers part of the top. A limited zone gave about 2,000 units in one sitting.
  • Two sittings still fall short of a full top for a limited zone, at about 3,600 units.
  • The crown competes with the front for every graft that is left.

These are group figures from one population, not a promise for any one person. Your own count is set only after the donor area has been measured.

How Does the Donor to Recipient Ratio Affect Density?

Density on top is the number of grafts divided by the area they cover. The same 3,000 grafts placed over the front third of the scalp look much denser than 3,000 grafts spread over the whole top. At Norwood 7 the donor band is small and the recipient area is large, so the ratio is poor.

That is why the plan narrows the target instead of thinning the grafts:

  • Cover a smaller zone well, starting with the front.
  • Accept a lighter mid-scalp and crown.
  • Use styling, a longer top or SMP to add the look of density where grafts cannot.

Can a Norwood 7 Be Done in One Session?

Rarely. Mohmand and Ahmad advised taking less than 35 percent of donor density in a first session and only 10 to 20 percent in a second, and noted that a donor area left with 20 to 30 percent of its original density looks odd. With a narrow band, those limits arrive quickly.

The NHS also notes that a large area may need two or more sessions on different days. For larger transplants, the 6,000 graft hair transplant guide explains what one big session involves.

How Is a Norwood 7 Transplant Planned?

Planning is about order. The supply is fixed, so each session has one job.

What Do Surgeons Prioritise?

The front frames the face and is what people see first. A dense front with a lighter crown looks natural, while a dense crown with a bare front does not. The 2021 guideline also calls the vertex a "black hole" for grafts, because the crown uses more of them than the front for the same visual result. A typical order is:

  • the hairline, set higher and softer than at a young age
  • the frontal third of the top
  • the mid-scalp, as far back as the supply allows
  • the crown, last, at lower density or not at all

Our guide to the mature hairline shows what a natural adult hairline looks like, and the crown hair transplant guide explains how the whorl is rebuilt when grafts allow.

When Is a Second Session Planned?

The sources we read give no fixed gap between sessions, so this guide does not state one. The first result has to grow out before it can be judged, and the donor area has to heal before it is checked again. The NHS puts the full result of a transplant at 10 to 18 months. The second session is smaller, because the donor area can give less the second time.

Can Beard or Body Hair Help a Norwood 7?

Yes, as an addition. The 2021 guideline says non-scalp donor areas should be evaluated, since taking too much from the safe donor area can ruin its look. For advanced grades and limited donor areas it describes combining methods, which needs expertise because of longer operating hours and a higher anaesthetic dose. The 580-man study counted about 824 beard units in its group.

Donor sourceWhat it addsLimit
Scalp bandThe main supply; hair matches the scalpNarrow at stage 7
BeardThick hairs that add bulk to the mid-scalp or crownNot for the hairline itself
Body hairExtra supply for very large plansFiner, can grow differently

Dua and Dua note that body hair can only be taken with FUE. Our guides to beard transplant and body hair transplant explain both sources in more detail. Whether you need them is decided after the scalp band has been measured.

Which Technique Suits a Norwood 7?

No technique adds hair to the donor area. The choice changes how grafts are taken and placed, not how many exist. FUE takes single follicular units and is the route to beard and body hair. DHI places grafts with an implanter pen and is mainly a placement method. The guideline also describes combining methods when donor supply is short.

At this stage the more useful questions are how far back the grafts are spread, how dense the front is and how much is kept for later. Our comparison of FUE vs DHI explains the differences.

Can Scalp Micropigmentation Help at Norwood 7?

Yes. Scalp micropigmentation (SMP) is a medical tattoo that places small dots of pigment in the scalp to look like shaved hair. A review of medical tattooing by Becker and Cassisi describes its aim as creating the illusion of hair follicles in areas of hair loss, including scars.

The review summarises a study of 80 men followed for over 6 months: 2 to 7 treatments each, 26 with at least one earlier transplant, and average satisfaction of 4.8 out of 5. The authors support scalp tattooing when loss is stable and transplant options are unavailable or used up.

For a Norwood 7, SMP can work in two ways:

  • Alone, for a shaved look without surgery.
  • With a transplant, to add the look of density behind transplanted hair or over donor scars. Garg and Garg describe it as an excellent non-surgical option to camouflage donor scars.

What Does a Successful Norwood 7 Result Look Like?

A good result looks natural from normal distance. It has a hairline that suits your age, a front and mid-scalp that cover the scalp in everyday light, and a crown that is lighter or left as it was. It does not look like the hair of a man who never lost any, and a plan that promises that should be questioned.

A Norwood 7 result is judged from a normal distance and in everyday light. Under bright light from above, a transplanted top will show more scalp than a full natural head of hair.

How Do You Read Norwood 7 Before and After Photos?

Before and after photos are among the most searched parts of this topic. Check these points:

  • Is it really stage 7? Look at the band. A wide, high band in the before photo means stage 6 or earlier.
  • Same angle and light? A front view in soft light hides what a top view under bright light shows.
  • Dry and styled? Dry, styled hair covers more scalp than wet hair.
  • How long after surgery? A result before 12 months is not final.
  • How many sessions and grafts? Ask what the result took.
  • Is SMP included? Pigment can add a lot of apparent density.

When Do You See Growth?

Growth follows the same slow pattern as any transplant. The NHS says transplanted hair often falls out after a few weeks, with new hair usually appearing after 4 months and the full result visible after 10 to 18 months.

Timeline of transplanted hair over 18 months from shedding to full results

The American Academy of Dermatology describes shedding at 2 to 8 weeks and results at 6 to 9 months, sometimes 12. With more than one session, each session starts its own timeline. Our recovery timeline guide covers each stage week by week.

What Happens Over the Long Term?

Transplanted hair comes from the donor band, where hair resists the hormone that shrinks hair in pattern loss. StatPearls notes that transplanted hairs keep the traits of the donor hair, which is why a transplant is expected to last. The 2021 guideline describes androgenetic alopecia as continuous and progressive, and recommends that patients keep up medical treatment after surgery, naming topical minoxidil 2 to 5% and oral finasteride 1 mg a day.

At Norwood 7 there is little native hair left on top, so the long-term questions are about the band:

  • It can thin with age, which is one reason a plan should never empty it.
  • Medicine helps to protect it, but only on a doctor's advice.
  • A lighter crown stays lighter unless more grafts are added later.

Our guide to how long hair transplants last covers this in more detail.

What Are the Risks?

The risks are the same as for any transplant, with one that matters most at this stage. The reviews by Kerure and Patwardhan and by Garg and Garg list these:

  • Swelling, redness and itching. Common and usually short.
  • Shock loss. Temporary loss of existing hair near the grafts.
  • Folliculitis and infection. Infection is uncommon.
  • Numbness in the donor or recipient area.
  • Small white scars where FUE grafts were taken, more visible with very short hair.
  • Overharvesting. Taking too much from a narrow band leaves it thin and patchy. At Norwood 7 this is the risk to ask about first.

Tell the team about any health conditions and medicines before surgery. Contact the hospital if you have fever, spreading redness, pus or severe pain.

Transplant, SMP or Neither?

A transplant is one choice among several at this stage.

OptionWhat it doesLimit
TransplantAdds real hair to the front and topLimited by a narrow band; several sessions
SMPAdds the look of shaved hairShaved look only; needs touch-ups
Transplant with SMPReal hair in front, the look of density behindTwo procedures
MedicineProtects the bandDoes not regrow a bald top
Shaved headNo procedureSome men prefer it; many famous bald men chose it

How Much Does a Norwood 7 Transplant Cost?

The cost depends on the number of grafts, the number of sessions, the technique and what the package includes. No price is quoted here, because a Norwood 7 plan is set only after your photos and donor band are reviewed. Our hair transplant cost page lists the packages and what each one covers.

Frequently Asked Questions

Can a Norwood 7 get a hair transplant?
Sometimes. It depends on the density, width and quality of the band at the sides and back. A doctor confirms it from photos and an examination.
Does a Norwood 7 transplant need a different type of anaesthesia?
No. The NHS says a transplant is usually done under local anaesthetic and sedation, so you are awake but feel no pain. If beard or body hair is added, the guideline notes longer hours and a higher anaesthetic dose.
Can SMP be combined with a stage 7 transplant?
Yes. SMP can add the look of density behind transplanted hair or over donor scars. In one study, 26 of 80 men treated with scalp tattooing had had a transplant before.
What is the maximum number of grafts taken in a single day?
There is no single safe maximum. A 2010 review described sessions of up to 2,000 grafts over 10 to 12 hours, and warned that pushing for more can mean taking hair from areas that may later go bald. At stage 7 the donor band, not the day, sets the limit.
How does the donor to recipient ratio affect density?
The same number of grafts looks denser over a smaller area. With a narrow band and a large bald top, the plan covers the front well rather than the whole top thinly.
Is there a sleeping position after a large transplant?
Yes. A review of complications by Garg and Garg says the usual advice is to lie flat on your back or slightly propped up, and to avoid sleeping on your side or face down. Your surgeon's instructions come first.
Can smoking or alcohol cause graft failure?
Smoking can. The reviews list heavy smoking among causes of poor growth and of small areas of skin damage in the recipient area, and advise stopping. The sources we used give no alcohol figures, so follow your surgeon's written instructions.
What is the ideal age for a transplant with total baldness?
The 2021 guideline advises waiting until at least 23 to 25, and does not recommend surgery under 21, because loss keeps progressing. Men at Norwood 7 are usually older, with a stable pattern.
How do surgeons prioritise grafts when full coverage is not possible?
Front first. The hairline and front third frame the face, the mid-scalp follows, and the crown comes last because it uses many grafts for a small visual change.
Is there more scarring with body hair (BHT)?
Body hair is taken with FUE, which leaves small round scars. Rose notes these are often lighter in colour and larger than the punch. The sources we used give no body-only scar rate.
When can I use hair fibres or concealers?
The sources give no fixed day. The NHS says grafts are not secure for the first 2 weeks, so nothing should be put on them then. Your surgeon gives the date.

Plan Your Norwood 7 Transplant

At Norwood 7, the plan matters more than the graft count. Send clear pictures from the front, the top, both sides and the back in daylight for a free hair analysis. The medical team will tell you what your donor band can support, what a first session would cover and whether SMP or a second session makes sense, and you can compare it with the hair transplant cost before you decide.
