Minoxidil is a hair loss medicine you either apply to the scalp as a liquid or foam, or take as a low-dose tablet. In a 48 week trial of 393 men with pattern hair lossGlossaryAndrogenetic alopecia, or pattern baldness, is the pattern hair loss behind most male and female thinning. It is primarily driven by genetic predisposition and sensitivity to androgen hormones. Open in the glossary →, the 5% liquid grew 45% more hair than the 2% strength and clearly beat placebo.[1] The regrowth lasts only while you keep using it.[2]
This entry covers how it works, how the two forms compare, what the first year looks like and where it fits with a hair transplant.
1Quick answerThe short answer, in a few sentences
What Is Minoxidil?
Minoxidil is a medicine for pattern hair loss. The liquid and foam are approved for this use, while low-dose tablets are prescribed off-label.[3] It pushes resting hair follicles into a new growth phase and may keep them growing longer.[4] The gain lasts only while you keep using it.[2]
It began as a blood pressure tablet. The tablet label still reserves it for severe high blood pressure that a diuretic plus two other drugs cannot control.[5]

Topical vs Oral Minoxidil: How It Works and What to Expect
How Minoxidil Works on Hair Follicles
In animal studies, minoxidil shortens telogenGlossaryTelogen is the resting phase of the hair cycle. It lasts 2 to 4 months according to a Deutsches Ärzteblatt review, and up to 4 months according to Cleveland Clinic. Open in the glossary →, the resting phase, so follicles start growing again early, and it probably does the same in people. It may also lengthen anagenGlossaryAnagen is the growing phase of the hair cycle. For scalp hair it lasts 2 to 6 years according to a Deutsches Ärzteblatt review, and 2 to 7 years according to Cleveland Clinic. Open in the glossary →, the growth phase, and enlarge the follicle, though the full mechanism is still unclear.[4]
Topical vs Oral Minoxidil Compared
In a trial of 90 men, 5 mg tablets taken daily for 24 weeks did not beat 5% liquid applied twice a day.[6]
Itch and scalp irritation, more with 5% than 2%[1]
Lightheadedness 1.7%, fluid retention 1.3%, fast heartbeat 0.9%[11]
A network meta-analysis of 23 studies found the largest gain in thick terminal hairs at 24 weeks with 5 mg tablets, ahead of both topical strengths.[12] A 2026 review credits the tablet with better adherence and fewer scalp reactions.[13]
How to Apply Topical Minoxidil
Start with dry hair and a dry scalp. If you have just shampooed, the UK leaflet suggests waiting one hour.[14]
Apply 1 mL of liquid, or half a capful of foam, to the thinning area and massage it in.[2][8]
Wash your hands, because product carried to other skin can grow unwanted hair there.[15]
Leave at least 12 hours between doses and use no more than 2 mL a day.[14] More product or more frequent use does not improve results.[2]
Minoxidil Results Timeline, Month by Month
Time on minoxidil
What the trials and labels show
Weeks 2 to 6
Some users shed more as old resting hairs make way for new ones; it usually settles within a couple of weeks[15]
First 12 weeks
In 49 patients on the liquid, extra shedding appeared in this window and lasted longer on 2% than on 5%[16]
Month 2
Men on 5% liquid had gained about 30 non-vellus hairs per cm², against 14 on placebo[15]
Month 4
The peak: 35.3 extra hairs per cm², against 15.3 on placebo; men who see nothing by now should ask a doctor[15][2]
Months 4 to 6
A response to tablets is expected; women on the foam may need 6 months[9][8]
Year 1
The gain had narrowed to 18.6 hairs against 3.9; about 60% of men had more scalp coverage on photos, against 23% on placebo[15]
New hair may come in soft and fine before it matches the rest.[14] If shedding lasts longer than two weeks, stop and see a doctor.[15]
Why Minoxidil Doesn't Work for Everyone
About 40% of people regrow hair on 5% topical minoxidil.[17] Response seems to depend on sulfotransferase, an enzyme in the follicle: in women on 5% minoxidil, its activity in plucked hairs predicted who would respond with 93% sensitivity.[18] Other reasons it can fail:
It is not pattern hair loss. The label rules out sudden or patchy loss.[15]
You stopped too early. Some men need at least 4 months.[2]
Minoxidil for Women
In a 24 week trial of 404 women, once-daily 5% foam grew 9.1 more hairs per cm² than placebo foam, with little irritation or facial hair.[19] Once-daily 5% foam and twice-daily 2% liquid grew almost the same, 23.9 and 24.2 hairs per cm² at 24 weeks.[20] Our female pattern hair loss entry covers other options.
Minoxidil Side Effects and Safety
Topical Side Effects
Only 1% to 2% of an applied dose reaches the blood.[15] Itch, rash and dermatitis are listed as common.[15] In 99 patch-tested allergy cases, minoxidil itself was the allergen in 74.7% and propylene glycol in 17.1%.[21] The foam contains no propylene glycol.[22]
Stop and ask a doctor if you notice any of these:[2]
Chest pain, a racing heart, faintness or dizziness
Sudden, unexplained weight gain or swollen hands or feet
Unwanted facial hair
Scalp redness or irritation
Oral Minoxidil Side Effects and Blood Pressure
In 1,404 patients on low-dose tablets, extra body hair affected 15.1%, and 1.7% stopped because of side effects.[11] A meta-analysis found no significant change in blood pressure, but heart rate rose by about 3 beats a minute.[23]
Rates of extra body hair vary widely: 15.1% in that series, 49% on 5 mg in a trial.[11][6] A 2026 review calls it dose-dependent and lists pericardial disease, uncontrolled high blood pressure and pregnancy as contraindications.[13]
Pregnancy, Babies and Pets
The US label says it may be harmful during pregnancy or breastfeeding.[2] Babies have grown extra hair after touching a caregiver's treated scalp.[15]
Minoxidil With Finasteride or a Hair Transplant
Minoxidil and Finasteride Together
In a 12 month trial of 450 men, 59% improved on 5% minoxidil, 80.5% on finasteride and 94.1% on both.[25] A meta-analysis also rated the combination above either drug alone, with similar safety.[26] Our finasteride entry covers that drug.
Adding microneedling improved hair counts in a meta-analysis of 12 trials.[27]
Minoxidil Before and After a Hair Transplant
Because it acts on existing follicles, minoxidil helps thinning hair rather than smooth skin.[4] Younger men, balding for a shorter time with a smaller bald patch on the crownGlossaryThe crown, also called the vertex, is the highest point at the back of the head, where hair grows out in a spiral. Crown and vertex are two names for the same area. Open in the glossary →, tend to respond better.[15]
A 2026 surgeon consensus says tablets can generally restart 1 to 3 days after a transplant, and topical minoxidil can go on the grafted area at 7 to 14 days.[9] Shedding after surgery is a separate issue, covered in shock loss after a hair transplant.
Frequently Asked Questions
Can minoxidil regrow a receding hairline?
The evidence is split. The US label says the 5% liquid is not intended for a receding hairline.[2] A 2026 surgeon consensus rated topical 5% effective at both the front and the crown.[9]
Is minoxidil a DHT blocker?
No. It is not a 5-alpha reductase inhibitor like finasteride or dutasteride.[12] Its main action is on potassium channels in the dermal papillaGlossaryThe dermal papilla is a cluster of cells at the centre of the hair bulb. It signals the bulb and helps decide how thick, how long and what colour each hair grows. Open in the glossary →.[13] Our DHT and hair loss entry explains the hormone.
Is foam or liquid minoxidil better?
In women, once-daily 5% foam grew almost as much hair as twice-daily 2% liquid.[20] The foam has no propylene glycol and gives about half the blood level of the 5% liquid.[22][15]
Is oral minoxidil prescription only?
Yes. Minoxidil tablets are prescription medicines, and their use for hair loss is off-label.[5][7] Experts advise checking weight and blood pressure before you start.[9]
Does minoxidil cause weight gain?
The UK label lists weight gain as a common effect of the liquid.[15] On tablets, fluid retention affected 1.3% of 1,404 patients.[11]
What happens if you stop using minoxidil?
The regrown hair may disappear 3 to 4 months after you stop.[15] In a study of 10 men, four fell below their starting hair count after stopping.[28]
Can I use minoxidil on my eyebrows?
The labels say to apply it only to the scalp.[2] What the brow studies found is in our entry on minoxidil for eyebrows.
Is Minoxidil Right for You?
Minoxidil suits men and women with pattern thinning who still have hair to keep and will use it every day, long term.[2] It is the wrong choice for sudden or patchy loss, which needs a diagnosis first.[15]
